A) Parents must name the medication, details of the dosage and frequency required in the box below.
B) Students will only bring the medication dose for the exact days of the Summer School. They will bring it in the original box with the information pamphlet(s).
C) Students will hand in the medication to the teacher/secretary who will keep it and administrate as and when stated in the information provided by parents (box below). By signing this form, you are giving your consent for staff to administer any agreed medication.
